This can be caused by overactive upper lip elevator muscles, including the levator labii superioris alaeque nasi, levator labii superioris, and zygomaticus minor, although other factors such as tooth position, gum anatomy, or jaw structure may also contribute. For patients whose gummy smile is primarily muscle-related, carefully placed micro-doses of botulinum toxin can temporarily relax these muscles, reducing the amount the upper lip lifts when smiling.
